1. Click Clients in the menu on the left-hand side of the screen
2. Click on your client's name in the Clients List.
Tip: You can also go directly to your Client's page by typing their name in the search bar at the top of the screen.
3. Click the New Request button on the top right-hand side of the screen.
4. Click the OHIP Request button on the top right-hand side of the screen.
5. You will then be asked to review their details to ensure that the selected Provider is the one you'd like to request records from. Once satisfied, click Select.
6. Select the date range for your summary. Please note that as per the Ministry's policy you can only request records as far back as 7 years from today's date.
7. Sign & Submit your request or Add it to Action Items.
