The Ministry of Health and Long-Term Care has created a new electronic process for requesting an OHIP Personal Claims History (PCH) or decoded billing summary. The Ministry of Health and Long-Term Care requires a new authorization to request PCHs moving forward. 


The new authorization is currently live in Medchart.

 

As of November 3rd, 2020, we will be updating our internal OHIP PCH request process to accommodate the Ministry of Health’s new authorization. There will be a few process changes, as well as a new authorization. Please review the new instructions below for requesting OHIP PCH moving forward.

 

New OHIP Request Process for Medchart

  1. Download the new OHIP patient form from the homepage of the portal or using this link: Download PHIP PCH authorization

  2. Have your client fill out the ENTIRE FORM (including the date and signature).

  3. DO NOT upload the filled-out form into the consent table. If you do, it will be rejected during the standard verification process.

  4. Proceed to make the OHIP request as usual. You will be prompted to upload the new authorization during the request process.

  5. Submit the request and Medchart will take care of the rest.


Once you click "Sign & Submit" you will be prompted to upload the signed OHIP form.


*E-signature is currently not available for OHIP PCH authorizations through Medchart. Your client can sign the form electronically through Adobe or Docusign on their own, and send it back to you, but e-signature will no longer be available through the Medchart portal for OHIP forms.
