Sick Kids is now accepting electronic signature along with photo ID and SDM documents! As of pre-COVID they only accepted original ink signed authorization forms, but now you may get your client to sign the Medchart authorization electronically and submit to us!
Upload any supporting documents and photo ID as you normally would to your Client's page in the MedChart portal. Once the client has signed the Medchart consent and photo ID / SDM documents are uploaded, you can go ahead and place your request to Sick Kids Hospital through our portal!
