Claim your Practice
Type in your clinic name or practitioner name in the search box.
Click "New Search".
If a listing matches your location, click "Claim".
Check the box "I accept the terms and conditions".
Click "Continue".
Note: If you cannot find your practice please scroll down to the "Can't find your practice?" section.
Fill in the requested information and choose a verification method. We will send you a verification code using the method you have chosen.
Click "continue".
You have successfully created an account.
Check your inbox for an email from Medchart.
Click on the link in the email to login.
Can't Find Practice
Select the first checkbox "Can't find your practice?"
Fill in the requested information on your practice.
Important: If you are a sole practitioner or you are registering as a sole practitioner in a larger organization, please check the appropriate checkbox.
Accept the terms and conditions.
Click "Continue".
Fill out the requested information in order to create an account.
Choose "mail" as your verification method. Medchart will mail you a verification code.
Click "Continue".
You have successfully created an account.
The next step is to verify and configure your account in order to activate it.
