A witness signature is needed on the Medical Information Release Authorization form before you are able to submit your request to a healthcare provider.


We require a witness signature to verify your identity on the request being submitted. This article will provide an overview of how to request a witness signature.


After you have created your request that needs to be submitted, you will navigate to My Request Cart to view your request. 


For the request you need to submit, you will see a purple pencil icon highlighted in yellow beside your request (on the right). This icon indicates that we require the consent form to be signed before proceeding with the request. 


1. Click on this button to proceed. 



A pop-window titled Witness Required will appear whereby you will need to enter the email and full name of a witness (i.e. a family member or a trusted friend). 


2. Enter their information and click Next.




Your witness will then receive an email with a link to DocuSign, enabling them to sign the consent form. 


3. The next window will be the consent form that you, as the requester, will be required to sign. Click Continue to proceed with signing the form electronically and follow the prompts. 



4. After you have signed the consent form, you will see that the icon beside the request has changed to a small purple person. This indicates that you are waiting for the witness's signature. It might take up to 30 minutes for the status to update here





5. Once the witness signature has been received, the status of the request will change to a greyed out check mark. This means that you are ready to place an order for this request. We have your valid consent and supporting documents on file. 





6. Please proceed to Go to Checkout to submit your request.
